How to Complete the F.O.I. Application Form
(Access/Correction Request)

Please ensure the following has been completed prior to submitting your request...

Fill in your date of birth.

Fill in your full name, address and telephone number.

Clearly describe what information you are requesting. List all particulars ( i.e. date, time, occurrence number, officer's name & badge number, name of other individuals involved, etc).

Sign and date the form.

Pay the $5.00 application fee for each request. A seperate form and fee must be submitted for each request.
